maybe it's because the name "plugins" is capitalized 'Plugins', put it in lowercase and check each plugin.

Go into your server.cfg file and make sure you have included all the plugins in the plugin section of that file.
So in your actual server folder, you should have a "plugins" folder, whatever plugins you have in the "plugins" folder make sure you add them inside server.cfg file.

I had same problem few days ago and this fixed it.

Look in your server.cfg, It says Plugins. Change it to plugins.

you have 2 times sscanf there, I don't think that's the problem, but I wouldn't have it here

The gamemodes/CODMW3.amx need be compiled with no error.
change in server.cfg from Plugins to plugins.
